The starting points of getting help from Skyview Counseling are...

1) Contact us and let us know what your counseling needs are.

2) Please read over and print the following "Professional Disclosure Statement". Please sign and date the last page once you have understood the material - especially the guidelines for confidentiality.

3) Fill out the following "Intake Forms" according to the type of counseling you desire. You may have to erase some lines and punctuation in order to type in your answers using Microsoft Word. Otherwise, if you are going to hand fill in the answers, please use a black ink pen to fill in your answers. Please write clearly and fill in all the places that apply to you.

· Individual Counseling

· Pre-Engagement and Pre-marital Counseling

· Groom's Intake Form

· Bride's Intake Form

· Marriage Counseling & Family Counseling

· Husband's Intake Form

· Wife's Intake Form

· Children's Intake Form (for children over the age of 12).

4) Then send both the last page of the "Professional Disclosure" paper (that you have signed and dated) and all the "Intake Forms" to Skyview Counseling via mail or fax. Try to make sure that the printing and margins look correct so that it will be easy for the therapist to read. (See "Contact Skyview" for fax numbers and the office address.) You may also send this information via Email if you have established a secure email account with Skyview and have a password for your account. Do not send any information over the Internet that is not secure!
